Open Question: Can someone help me make an invoice for a web-design business?

I have recently made a website for a company and I want to send an invoice for the finished work. I don't have an invoice and all the templates I can find on the net aren't really appropriate. Can someone either send me an example of what it should look like or thoroughly explain what I need on there.
